Hair transplant aftercare: the post-operative instructions

By Klineva · Updated on 5 October 2026 · 4 min read

Here are Klineva’s instructions after a hair transplant in Istanbul: what to do, what to avoid, and for how long. They cover hydration, physical activity, the sun, smoking and alcohol, moisture and swimming, haircuts and dyeing, sleep and protecting the transplanted area. A summary then groups them by duration. Washing your hair has its own page: Washing your hair after a transplant. If you have any questions, our advisers are here to help.

A very loose white linen bucket hat, an aqua-green neck pillow, a bottle of water and a white towel on a light oak table by a window: the essentials for the first days after a hair transplant

Which instructions should you follow after a hair transplant?

Hydration

Three glass bottles filled with water and a glass of water with a slice of lemon and mint, on a light oak table: drink plenty of water after a hair transplant

Drink plenty of water in the first few days (3 to 5 litres a day) to avoid swelling. Also avoid overly salty foods for 3 to 5 days.

Walking, exercise and sexual intercourse

A pair of white and aqua-green trainers in a bright hallway, next to a light oak bench: walking is possible from the first week
  • Walking – possible from the first week, without your scalp sweating.
  • Exercise – no physical activity for 1 month, yoga and Pilates included. You then return to it gradually.
  • Sexual intercourse – none for 15 days.

Sun

A very loose white linen bucket hat on a light oak table, next to a glass of water, under a large aqua-green parasol on a sunny terrace: protect yourself from the sun after a hair transplant

The first 15 days are the most important: avoid any sunburn and protect yourself. The clinic provides a very loose bucket hat that does not touch the transplanted area: you can wear it, provided you take care. To protect yourself from the sun or rain, use an umbrella rather than a hat.

Sunburn will damage your grafts!

High-risk sports

A pair of turquoise boxing gloves and a white ball on a light oak bench, next to a folded towel: no combat sports or ball sports for 1½ months

Do not practise high-risk sports, such as combat sports and ball sports, for 1½ months.

Smoking and alcohol

A glass of water and a cup of mint tea on a light oak tray, with an empty wine glass and an empty ashtray in the background: no smoking or alcohol after a hair transplant

Do not smoke (cigarettes, e-cigarettes, shisha…) for at least 3 days. Do not drink alcohol while you are taking your medication (until about day 7 after surgery), and above all not with antibiotics.

Moisture, swimming pool, sea and sauna

A light oak stool at the edge of an indoor swimming pool, with a rolled white towel, swimming goggles and a seashell, in front of the glass door of a sauna: no swimming pool, sea or sauna for 1 month

Avoid swimming pools, the sea, saunas and hammams for 1 month. Also avoid moisture and sweating for 1 month (apart from the prescribed washes), then resume gradually.

Haircuts and hair dye

Hairdresser’s scissors, a turquoise comb, a bowl with a hair-dye brush and a folded white towel on a light oak shelf: haircuts and hair dye have to wait after a hair transplant

No haircut with scissors before 3 months. For clippers and hair dye, wait 6 months.

How should you sleep and protect the transplanted area?

  • To sleep or rest, use the neck pillow provided by the clinic.
  • Without a pillow to hand, roll up a bath towel and tuck it into the hollow of your neck.
  • The freshly transplanted area must not touch the bedding for the first 7 days.
  • Sleep on your back, with the neck pillow, and not on your stomach for at least 1 week.
  • On the first night, a dressing protects the donor area, sometimes with a headband as well. It is removed at the first wash, at the clinic (day 1 after surgery).
  • Do not wear a cap, a beanie or a helmet (building site or motorbike) for 1½ months.
  • Do not wear a sleep cap for 1 month.
  • Do not touch or scratch your grafts. Wash your hands thoroughly to avoid any risk of infection.
  • Do not bend forward (head down) for 5 days.
A bed with white sheets, an aqua-green neck pillow on the pillow and a turquoise throw, next to a light oak bedside table: sleeping well after a hair transplant

Sleeping positions are explained in detail in the guide Sleeping after a hair transplant.

For 15 days

  • Headwear strongly discouraged (apart from the bucket hat provided by the clinic)
  • No sexual intercourse
  • Be careful with the weather conditions (UV, sun, rain)
  • A healthy lifestyle is recommended, in particular washing your hands thoroughly to avoid any risk of infection

For 30 days

  • No exercise, yoga and Pilates included (walking is possible from the first week, without your scalp sweating)
  • No moisture or sweating (apart from the prescribed washes)
  • Do not wear a helmet
  • No swimming in the sea or in a pool, no sauna or hammam
  • No sleep cap
A plate of grilled salmon with broccoli and green beans, a lemon wedge and a glass of water: a protein-rich diet after a hair transplant
ReminderNo alcohol while you are taking your medication (until about day 7 after surgery), and above all not with antibiotics. A healthy lifestyle and a good diet, rich in natural proteins (fish, meat, vegetables…), are strongly recommended.

Swelling, scabs, shedding of the transplanted hairs: all of this is described in the guide The first days after a hair transplant, from day 1 to day 30 after surgery.
