Sleeping after a hair transplant: which position and which pillow?

By Klineva · Updated on 5 October 2026 · 7 min read

After a hair transplant, the best sleeping position is on your back with your head raised. Also avoid letting the transplanted area rub against the pillow or the sheet, because the grafts are still fragile. Good sleep also helps your body recover and cope better with the stress of the operation. This guide explains how to lie down, when you can sleep on your stomach, which pillow to choose and how to relax. It then recaps the essentials of aftercare, with links to our other guides.

Why is good sleep important after a hair transplant?

Sleep is an important factor in recovery after a hair transplant. Sleeping gives the body time to rest and recover. Good-quality sleep also supports healing. So do not neglect your hours of sleep.

Sleep also helps you manage stress better. A hair transplant using the FUE or DHI can be a source of anxiety. Restorative sleep helps reduce it. Post-operative discomfort is also often eased by a good night’s sleep.

Which position should you sleep in after a hair transplant?

For the first 7 days after your hair transplant, it is crucial to keep the transplanted area from coming into contact with the pillow or the sheet. The grafts, known as follicular units, are still fragile. They must be protected from any knock and any rubbing.

The best sleeping position is therefore on your back, with your head raised. If you find it hard to get used to this position, use pillows to support your back. They stop you from turning onto your side or your stomach.

When can you sleep on your stomach or as normal after a hair transplant?

As a rule, you should wait at least a week before sleeping on your stomach. This always depends on the area where you received the hair grafts. Until then, sleep on your back, using the neck cushion.

On the first night, a dressing protects the donor area, sometimes with a headband as well. It is removed at the first wash, at the clinic (day 1 after surgery). In every case, it is essential to take care that the grafts are not damaged. Your medical team will support you through this stage.

Do not sleep on your stomachSleeping on your stomach puts considerable pressure on your head and scalp. This pressure can dislodge the grafts and compromise the results of the transplant. Avoid sleeping on your stomach for at least a week after the operation: sleep on your back, using the neck cushion.

Which pillow or cushion should you sleep on after a hair transplant?

After a hair transplant, it is strongly recommended to sleep with a special pillow. By raising your head, you can improve blood circulation, which helps healing. It also helps you stay in a semi-seated position and avoid pressure on the recipient area.

U-shaped or memory-foam pillows give more support to the neck and head. They therefore reduce the pressure on the grafts. At Klineva, we provide a neck cushion. In the first few days, it holds the head in place so that the transplanted area does not touch the bedding. If you do not have one, a rolled-up bath towel placed in the hollow of your neck can replace it.

The grafts remain vulnerable after the operation. So avoid scratching or touching them while you sleep. Be gentle with your scalp while you rest: this is essential to protect the grafts properly.

How can you fall asleep more easily after a hair transplant?

Also think about the environment in which you are going to rest. Preferably choose a quiet, calm room that is conducive to relaxation. To create a soothing atmosphere, cut out background noise and turn off all the lights. If necessary, you can use relaxation techniques, such as deep breathing or meditation. If you need support, do not hesitate to ask your clinic for advice.

What hair care do you need after a transplant, besides sleep?

Sleep is only one part of recovery. Before applying any product to your hair, ask your surgeon for instructions: your surgeon will advise you based on your situation. For washing, follow the clinic’s protocol, set out in our guide Washing your hair after a transplant.
